Phyliss Ann Swartz

Phyliss Ann Swartz, age 70, of Toledo, OH, passed away at St. Vincent Mercy Medical Center in Toledo, OH, on August 10, 2024. She was born on January 24, 1954, in Oregon, OH, to Stephen A. and Colice C. (Bray) Kollarik. Phyliss would meet and marry Steven J. Swartz and raise four children together. Phyliss and Steven celebrated many years of marriage together until Steven’s passing in July of 2022.

Phyliss was a registered nurse for over 40 years at St. Charles Hospital and the Luther Home of Mercy. She was a fan of musician Alice Cooper, comedian Jeff Dunham, and actor Johnny Depp. Phyliss also enjoyed Words with Friends and enjoyed watching NCIS. Phyliss loved her children and her grandchildren. She was a fiercely loyal friend, was known to be opinionated, and will be missed by all those who knew and loved her.

Left to cherish her memory are children, Aaron, Adam, Matt (Jamie), and Carrie (Joe); grandchildren, April, Jet, Wyatt, Anthony, and one grandchild on the way; siblings, Mary, Janice, Gilbert, Nancy, Paula, and Louie. In addition to her husband, she is preceded in death by her parents and other siblings.

Family and friends may visit the Freck Funeral Chapel , 1155 S. Wynn Rd in Oregon, on Friday, August 16, 2024, from 2:00 p.m. until 8:00 p.m. The Funeral Mass will be held at 10:00 a.m. on Saturday, August 17, 2024, at Epiphany of the Lord Parish-St. Thomas Aquinas Catholic Church, 729 White Street, Toledo, OH 43605, with the family to receive guests after 9:00 a.m. Burial will follow at Ottawa Hills Memorial Park.
